Video Summary
This session focused on the growing role of autonomic modulation in electrophysiology, especially for recurrent arrhythmias and VT storm. The panel agreed that while ablation and drugs remain central, many patients still have unmet needs, particularly when traditional therapies fail or are not feasible.

Shiv Kumar emphasized that the autonomic nervous system is fundamental to cardiac function and arrhythmogenesis, affecting conduction, contraction, metabolism, and vascular control. The group discussed how sympathetic overactivation can be targeted through beta-blockers, sedation, stellate ganglion block, surgical sympathectomy, renal denervation, and newer catheter-based or ultrasound approaches.

A major theme was the challenge of measurement: existing noninvasive markers like heart rate variability are limited, and better biomarkers are needed to confirm and guide therapy. Human and animal studies were described showing that stimulation or ablation of autonomic targets can provoke measurable changes in heart rate, blood pressure, and even arrhythmias.

The discussion also highlighted the promise of more precise, reversible neuromodulation methods, including electrical and chemical modulation, rather than purely destructive ablation. The panel concluded that autonomic therapies may soon move from bailout options toward earlier adjunctive or even replacement strategies in selected patients.
